Instructions for Gift of Securities

1. Transfer the asset to DTC #0164 (code 40) at Charles Schwab to account #4116-2266 Cincinnati Playhouse in the Park.

2. Contact account representative Marcy Mullins with any delivery-related questions at 513/621-4612, extension 540.

3. To ensure the donor is properly credited for the gift, the transferring broker should include in the DTC the NAME of the donor and the PURPOSE for which they are making the donation.

4. Please notify the Playhouse Development Assistant, Kathy Selby, by phone at 513/345-2242, extension 2025, fax at 513/345-2254, e-mail at kathy.selby@cincyplay.com or letter. Identify the name of the donor and the number and type of securities being transferred. (For your tax purposes, it is imperative that your broker contact the Playhouse with stock type, number of shares traded and donor name before transferring the gift.)
